Can I have a former name removed from the College’s public register?

Yes, you can request to have your former name removed if:

• you have never taught under the former name, you can request for it to be removed by completing the Request to Remove Former Name from Public Register form;

OR

• there are extenuating circumstances such as a threat to your personal safety, reasons of personal dignity, including gender identification or gender expression. If this applies to you, please provide a signed letter detailing the name(s) you would like withheld, the reasons why and your application/registration number. Additional supporting documentation may be required.
